MEMORANDUM
TO: All Businesses/Community Partners

RE: Proof of Vaccination Exemption for Business and Workplace
Based on provincial direction, businesses and employers could require employees, contractors, and visitors who wish to access the Businesses/Workplace to provide a proof that they are fully vaccinated.
The criteria for a medical exemption from COVID-19 vaccination are prescribed by the Ontario Ministry of Health and can only be signed off by a physician or Registered Practical Nurse. There are only two valid medical exemptions from COVID-19 vaccinations:
• an allergic reaction to a component of the vaccine within an individual, which must be confirmed by an allergist or immunologist, OR,
• if second dose delayed due to an individual having suffered myocarditis or pericarditis after the first dose of a vaccine.
In the event of a request for exemption on the grounds of a medical reason, the business/employer has the right to request an independent medical assessment be performed and appropriate documentation be received.
Sincerely,
Dr. Ian Arra
Medical Officer of Health
